Watch the interesting US Institute of Safe Medication Practices video on a now obsolete insulin pen — which had problems as highlighted in Thimbleby’s video and article. (It looks like the manufacturers decided to stop making the consumables so the Opticlik naturally became obsolete; this would certainly have been better PR than a recall.)

I argue that safety critical systems - such as healthcare systems - are too complex to be left to conventional usability studies alone. The article uses the Fluke 114 as a case study.
